Primary Care Partnerships are voluntary alliances of primary care providers usually covering two or
three local government areas.
Primary Care Partnerships aim to improve the health and well-being of their catchment’s population by
better coordination of planning and service delivery in response to identified needs.
The planning and delivery of services by Primary Care Partnerships will be underpinned by a ‘social
model’ of health, which takes account of the full range of societal, economic, environmental and
biomedical factors that influence people’s health and well-being.
